4. Do you have any social, economic, physical, and or psychological problems that could be improved by weight loss?
5. Is your obesity caused by a metabolic or endocrine disorder?
6. Do you have a history of substance abuse?
7. Do you have any serious medical problems that would make surgery dangerous?
8. Are you a woman who is thinking about or may become pregnant in the near future?
